Etidronic acid, also known as etidronate, is a bisphosphonate used as a medication, detergent, water treatment, and cosmetic.

 

It was patented in 1966 and approved for medical use in 1977.[1]

Etidronic acid is a bisphosphonate used to strengthen bone, treat osteoporosis, and treat Paget’s disease of bone.

 

Bisphosphonates primarily reduce osteoclastic activity, which prevents bone resorption, and thus moves the bone resorption/formation equilibrium toward the formation side and hence makes bone stronger on the long run. Etidronate, unlike other bisphosphonates, also prevents bone calcification. For this reason, other bisphosphonates, such as alendronate, are preferred when fighting osteoporosis. To prevent bone resorption without affecting too much bone calcification, etidronate must be administered only for a short time once in a while, for example for two weeks every 3 months. When given on a continuous basis, say every day, etidronate will altogether prevent bone calcification. This effect may be useful and etidronate is in fact used this way to fight heterotopic ossification. But in the long run, if used on a continuous basis, it will cause osteomalacia.

HEDP is used as a retardant in concrete, scale and corrosion inhibition in circulating cool water system, oil field and low-pressure boilers in fields such as electric power, chemical industry, metallurgy, fertilizer, etc. In light woven industry, HEDP is used as detergent for metal and nonmetal. In dyeing industry, HEDP is used as peroxide stabilizer and dye-fixing agent; In non-cyanide electroplating, HEDP is used as chelating agent. The dosage of 1-10 mg/L is preferred as scale inhibitor, 10-50 mg/L as corrosion inhibitor, and 1000-2000 mg/L as detergent. Usually, HEDP is also used together with polycarboxylic acid(superplasticizer),in which it acts as reducing agent.

 

 

Chelating agent and antioxidant

Etidronic acid is a chelating agent and may be added to bind or, to some extent, counter the effects of substances, such as calcium, iron or other metal ions, which may be discharged as a component of grey wastewater and could conceivably contaminate groundwater supplies. As a phosphonate it has corrosion inhibiting properties on unalloyed steel. Etidronic acid also acts to retard rancidification and oxidation of fatty acids.

 

HEDP and its salts are added to detergents and other cleaning agents to prevent the effects of hard water. It is also used in peroxide bleaching to prevent degradation of peroxides by transition metals.

Etidronic acid is listed as an ingredient of several cosmetic formulations where it is used for suppressing radical formation, emulsion stabiliser and viscosity control. While etidronic acid has not been limited from inclusion in cosmetics and does have legitimate uses, it is recommended that, as with most cosmetic products (particularly soaps), the product should be thoroughly rinsed from the skin after use.

Etidronic acid is also included among swimming pool chemicals. It is used as a stain inhibitor to prevent metal ions coming out of solution and staining the sides of swimming pools.

 

Synthesis

Etidronic acid can be prepared by reaction of Phosphorus trichloride with acetic acid in a tertiary amine, or by reaction of an acetic acid/acetic anhydride mixture with phosphorous acid.[3]

Etidronic acid is only found in individuals that have used or taken this drug. It is a diphosphonate which affects calcium metabolism. It inhibits ectopic calcification and slows down bone resorption and bone turnover. [PubChem]Bisphosphonates, when attached to bone tissue, are absorbed by osteoclasts, the bone cells that breaks down bone tissue. Although the mechanism of action of non-nitrogenous bisphosphonates has not been fully elucidated, available data suggest that they bind strongly to hydroxyapatite crystals in the bone matrix, preferentially at the sites of increased bone turnover and inhibit the formation and dissolution of the crystals. Other actions may include direct inhibition of mature osteoclast function, promotion of osteoclast apoptosis, and interference with osteoblast-mediated osteoclast activation. Etidronic acid does not interfere with bone mineralization. In malignancy-related hypercalcemia, etidronic acid decreases serum calcium by inhibiting tumour-induced bone resorption and reducing calcium flow from the resorbing bone into the blood. Etidronic acid also reduces morbidity of osteolytic bone metastases by inhibiting tumour-induced bone resorption. Etidronic acid may promote osteoclast apoptosis by competing with adenosine triphosphate (ATP) in the cellular energy metabolism. The osteoclast initiates apoptosis and dies, leading to an overall decrease in the breakdown of bone.

L’acide étidronique (DCI) ou acide 1-hydroxyéthane 1,1-diphosphonique (HEDP) est un bisphosphonate utilisé comme agent de chélation du calcium, dans le traitement de l’eau, les produits cosmétiques ou comme composé pharmacologique.

 

On trouve L’ acide étidronique le plus souvent sous la forme d’étidronate, sel de l’acide étidronique, abrégé en MnHEDP (M: cation, n allant jusqu’à 4).

Les constantes de protonation de HEDP et ses constantes de complexation avec Cu(II), Zn(II), Ni(II) et Cd(II) ont étés déterminées par titrage acide-base à 25 ± 0,5°C à force ionique 0,1 mol.L-1 (KNO3), gr[acaron]ce aux programmes PKAS et BEST de Martell et Motekaitis. Les complexes peuvent ětre classés par ordre de stabilité comme suit: Cu > Zn > Ni > Cd pour des valeurs de pH supérieures á 4. Les complexes peu solubles de HEDP avec le plomb et le cadmium ont été étudiés par potentiométrie et polarographie; les espèces CH3-C(OH)(PO3)2Pb2 et CH3-C(OH)[PO2(OH)]2Cd ont été mises en évidence et leurs produits de solubilité’ ont été évalués.

 

 

Mécanisme d’action

L’acide étidronique a un effet antiostéoclastique et ralentit le remaniement osseux (action inhibitrice sur la résorption osseuse démontrée expérimentalement chez le rat et la souris).L’acide étidronique diminue l’hydroxyprolinurie et le taux de phosphatases alcalines sériques. L’acide étidronique redonne au tissu osseux sa structure lamellaire et prévient la calcification des tissus mous (chez le rat).

 

L’acide étidronique a un effet antalgique sur la douleur pagétique.

Abstract

L’acide éthane hydroxydiphosphonique (EHDP) ou acide étidronique, qui est commercialisé sous le nom de Didronel ® , et dont l’indication principale est l’inhibition de la résorption osseuse dans la maladie de Paget, pourrait avoir une nouvelle application en radiotoxicologie. A cet égard, les travaux présentés portent sur son action décontaminante dans les radiocontaminations cutanées engendrées par l’uranium et le plutonium. Cette efficacité est mise en évidence par deux techniques in vitro utilisant des explants cutanés humains et qui sont habituellement exploitées en dermopharmacologie pour évaluer l’action des médicaments à visée topique.

L’acide étidronique est utilisé en tant qu’agent chélateur dans les cosmétiques. Il crée des complexes avec le calcium, l’arsenic, le fer et autres ions métalliques pour les neutraliser. Cela permet de gérer l’utilisation d’eaux un peu “dures”, qui pourraient interférer avec les tensioactifs du produit par exemple. Les chélateurs sont souvent utilisés dans les adoucisseurs d’eau. Cette molécule ne pose pas de problèmes pour la santé humaine, son inconvénient est que, tout comme l’EDTA, elle est très peu biodégradable, et les complexes qu’elle forme avec les métaux se retrouvent éparpillées dans la nature.

Utilisation de L’ acide étidronique

Pharmaceutique

L’ acide étidronique (marque Didronel ) est un bisphosphonate utilisé pour renforcer l’ os , traiter l’ ostéoporose , et de traiter la maladie osseuse de Paget .

 

Les bisphosphonates réduisent principalement ostéoclastes activité, ce qui empêche la résorption osseuse , et se déplace ainsi l’équilibre de la résorption osseuse / formation vers le côté de la formation et rend donc les os plus forts sur le long terme. Étidronate, contrairement à d’ autres bisphosphonates, empêche également l’ os calcification . La dose de 10.1 mg / L est préféré comme inhibiteur de tartre, de 10 à 50 mg / L en tant qu’inhibiteur de corrosion, et de 1000 à 2000 mg / L en tant que détergent. Habituellement, HEDP est également utilisé conjointement avec de l’acide polycarboxylique (superplastifiant), dans laquelle il agit comme agent réducteur.
